Bolus Use of Pressors

Two pressors come to mind when you think about bolus use- Epinephrine and Phenylephrine

Phenylephrine is the more commonly used agent- it is a pure alpha agent.  Its onset of action is about 1 minute and it lasts about 20 minutes.

You can administer 50-200 mcg every 2-5 minutes as needed while you are attempting to get central access.

EMCrit has a good little blurb which I have cut and pasted below to show how to mix it up.

Mixing Instructions:

Take a 3 ml syringe and draw up 1 ml of phenylephrine from the vial (vial contains phenylephrine 10 mg/ml)

Inject this into a 100 ml bag of NS

Now you have 100 mls of phenylephrine 100 mcg/ml

Draw up some into a syringe; each ml in the syringe is 100 mcg
